Appropriate Referrals to SKY

Diagnosis of a Neurological Injury:

Stroke (Cerbrovascular Accident-CVA)
Acquired Brain Injury ( Aneurysm, Anoxia, Brain Tumor Removal)
Traumatic Brain Injury
Multiple Sclerosis
Guillan Barre
Spinal Cord Injury with residual cognitive deficits.
Other Neurological Conditions

Age 16 or older

Expected to Make Functional Improvement:

Follows at least one step commands
Able to physically and mentally able to participate in therapy
Requires two or more therapy services

Financially Eligible:

Commercial Insurance (AETNA, CareFirst MAMSI/United Healthcare, TRICARE, etc)
Managed Medicaid
Medicare
Workers Compensation
Private Pay (Personal Checks, MasterCard/VISA)
