Clinical Care Coordination
Clinical Care Coordination is provided to ensure there is a main point of contact for the SKY Clients and Families and any parties involved in the client’s care. Assistance with acquiring resources, for example, durable medical equipment referrals to medical specialists, SSDI, etc and assistance with discharge planning and community reintegration are also coordinated. Family observation days or conferences are also facilitated to promote carry over of treatment and strategies within the home environment.
